Each homework assignment must have the following format.

1.  All problems must be done in the order assigned.
2.  Staple the pages in the upper left corner.
3.  The assignment sheet must be stapled to the top of your answers.
4.  No answers should be written on the assignment sheet.
5.  There will be penalties for doing problems out of order and for
     writing on both sides of the paper.
6.  To receive full credit, all work should be shown.  Answers to
     True-False or other short answer questions must be justified.
